PREFACE This book presents comprehensive coverage of new techniques, important developments, and innovative ideas in all fields of clinical dentistry. Topics discussed herein include indirect composite restoration in the posterior region; fissure sealings with resin modified glassionomer cement; craniofacial anthropometry; oral cavity overinfection during non-surgical cancer therapies; numb chin syndrome; and the growing need for collaboration between dentistry and medicine. Chapter 1 - The dental profession is unique among the various types of health care providers since in most instances, only the dental practitioner is responsible for the diagnosis process, determination of treatment options, and performing the chosen treatment. Variations have been found among dental practitioners regarding diagnosis and treatment in the various fields of dentistry. This article reviews the educational factors influencing the clinical decision making and treatment planning in the dental practice. As a part of the first issue of the new International Journal of Clinical Dentistry, it is believed that continuing education, exchange of opinions as well as clinical research in a multidisciplinary forum for publications from all fields of oral medicine is of utmost importance to bridge basic and clinical sciences, promoting the exchange of information and the advancement of oral medicine for the benefit of patients. Chapter 2 - The recent emergence of highly pathogenic H5N1 strains and the occurrence of human cases of H5N1 infection have prompted predictions of a pandemic-scale avian influenza viral infection among humans. Highly pathogenic H5N1 avian influenza viruses have become endemic among birds throughout the world, owing to the rapid carriage of H5N1 by migrating birds around the globe. In a relatively short period of time, H5N1 has acquired the ability to infect more mammalian species with fatal outcomes than any other influenza virus. So far, H5N1 has spread to more than 12 countries and its mortality rate reportedly exceeds 50%. It is difficult to predict when, whether, or which type of H5N1 influenza A virus will acquire efficient human-to-human transmission and lead to a pandemic, and the most important public health strategy at present therefore is to prevent the spread of the virus through the education of both health care workers and the general public. In this review, the authors summarize the current knowledge in H5N1 virology, its historical and epidemiological background, and clinical and hospital management strategies including those appertaining to dentistry. Chapter 3 - Purpose: To evaluate the clinical performance of two self-etching adhesives and one total-etch adhesive in Class V carious lesions.

Methods: Two self-etching primers, ‘Clearfil SE bond’ and ‘Hybrid Bond’ and one total etch adhesive, ‘Admira bond’, were placed in 244 Class V carious lesions. Restorations were evaluated at base line, one and two year using USPHS criteria. Results: No loss of restorations was recorded after one and two year for all materials. There was no significant difference between the base line and two-year results for Admira bond and Clearfil SE bond restorations. In contrast, restorations of Hybrid bond showed significant deterioration in marginal adaptation and cavosurface marginal discoloration after two years. No significant differences were found between Admira bond and Clearfil SE bond at each recall period. Clinical Significance: Total-etch adhesive (Admira bond) and two-step self-etch adhesive (Clearfil SE Bond) when placed in Class V carious lesions showed excellent clinical performance after two years. In contrast, one step self-etch adhesive (Hybrid Bond) showed significant deterioration with regard to the marginal adaptation and marginal discoloration. Chapter 4 - The site-specific nature of the volumetric features and enzymatic profile of gingival crevicular fluid (GCF): Analysis of myeloperoxidase (MPO) activity at distinct sampling sites. Purpose: Further clarification of the biodynamics and the site-specific nature of gingival crevicular fluid (GCF), in terms of volume and content, may serve both for a better understanding of periodontal pathogenesis and developing reliable diagnostic tools. Thus, the aim of the present study was to analyze, on a site-specific basis, the potential variances in GCF volume and its myeloperoxidase (MPO) content and the relationship of these two measures with the clinical periodontal status. Methods: At a total of 719 sites, representing either clinical periodontal health (n=175), or gingivitis (n=308), or periodontitis (n=236), clinical parameters were recorded, GCF samples were obtained and MPO activity was spectrophotometrically determined. Statistical analyses considered the comparison of the three periodontal conditions, multi-rooted or single-rooted teeth, and mesio-buccal (MB) and disto-buccal (DB) sites, and the correlations among the clinical parameters, GCF volume and MPO activity. Results: There were clear differences between all site-analysis and analysis of distinct sites. For all sites clinical parameters, GCF volume, and GCF MPO activity generally presented with a disease-related spectrum; healthy
